Using Team-Based Care to Improve HIV Linkage to Care
Available On Demand: Webinar Recording and Slides
ABOUT
Using a team-based approach can significantly improve HIV linkages to care. This training will address why having different components of a multidisciplinary team, when working together to provide HIV services to a suburban community, can be very advantageous and improve care delivery. How to foster effective communication and collaboration among multidisciplinary healthcare teams, including physicians, nurses, specialists, pharmacists, social workers, and other stakeholders, to exchange critical information, insights, and expertise is also essential. The training will also include an overview of how to streamline daily healthcare workflows and processes by optimizing task allocation, reducing redundancies, and eliminating barriers to information sharing. The speakers will review the outcomes of a successful Ryan White Program.
